ADVANCE REGISTRATION
To qualify for advance registration discounts, online registration must be completed
by 11:59 PM Pacific Daylight Time, Wednesday, August 3; registration forms and payments by mail must
be received by Wednesday, August 3. Beginning August 4, the on-site rates will apply.
REGISTRATION FEES
It is permitted to register for Tutorials, Conference, and Workshop separately or
in any combination of the three.
CONFERENCE REGISTRATION includes
admission to the technical and poster sessions, keynote and invited talks, opening wine and cheese reception
(Aug. 8), breakfasts, lunches (Aug. 9-11), dinners (Aug. 9-10), daily refreshment breaks, and eligibility
to use the Stanford student housing, the SLAC guest house, and discounted rates at the Cardinal, the
Sheraton Palo Alto, and the Westin Hotels in Palo Alto. Conference registrations
received after July 15 are not guaranteed a hard copy of the Proceedings, but will receive a CD version of
the Proceedings.
Guest tickets for lunch and dinner will be available at the on-site registration desk on a space-available
basis, at an additional charge.
The dinner on Wednesday August 10 is the conference banquet to be held at the Stanford University Faculty Club. Attendance is limited.
If you want to attend, please select this event as part of your registration package. Onsite tickets for this event will be on a
space-available basis only.

TUTORIAL REGISTRATION includes ONE morning and ONE afternoon tutorial as reserved when you
register, one set of lecture notes for the tutorials attended, breakfast, snack breaks, lunch and

MEMBER DISCOUNTS
If you claim the Member Discount, please be sure to fill in your IEEE Member Number on the
Registration Form, and please bring your current membership card with you. If you are not currently a member,
you can enjoy discounted registration rates by applying for
IEEE, or
IEEE Computer Society membership now.
STUDENT DISCOUNTS
Student discounts are available only to students enrolled in accredited degree programs
at the undergraduate or graduate level. We regret that postdocs and attendees at continuing education or
extension programs, or correspondence schools are not eligible to receive student discount rates. If you
claim the student rate, please specify the institution that you are attending on the registration form, and
bring with you evidence of current enrollment (Spring, Summer, or Fall Term 2005).
WIRE TRANSFERS
Attendees can register in advance and pay by wire transfer for an additional processing
fee of $50 per wire transfer. Wire transfers must be received by the 11:59 PM Pacific Daylight Time,
Wednesday, August 3, 2005 deadline. Please contact

TRAVEL VISA POLICIES
Authors and attendees of IEEE conferences are responsible for obtaining visas from the
host country they are planning to visit. Travelers should check the host country's policies, and if a visa
is required, obtain it from the appropriate foreign consular representative before proceeding abroad.
Applicants for visas should generally apply at the embassy or consulate with jurisdiction over their place
of permanent residence, as it may be more difficult to qualify for the visa outside their country of permanent
residence. Once received, visas should be reviewed to ensure there are no mistakes. Processing and visa fees
vary, and fees may not be refundable.
With security policies tightening around the world, adequate time should be allowed for processing visa
applications. Applicants should initiate the process as early in their travel planning as possible.
The embassy or consulate of the conference's host country should be consulted for specific details.

INVITATION LETTERS
Conference registrants who require invitation letters from CSB2005 in
order to process their visa requests should correspond
with us via the e-mail visa-letters@csbcon.org or via FAX to
+1-650-941-2015. Letters will only be sent to the individuals who have
registered for the conference, and not to any third parties.
Please note that no letters can be sent until after we have received
and verified the payment of all registration fees. Please make
requests early enough to allow time for delivery of our letter via
Postal Service or FAX prior to scheduled interviews.
REGISTRATION CONFIRMATION
Your on-line registration will be confirmed by email as soon as your credit card
company approves the charges. Fax or postal registrations will be confirmed within 48 hours of receiving
your registration. If you do not receive confirmation, please email us at

PAYMENT
Payment must accompany all registration forms and can be made by credit card (MasterCard,
VISA, Discover, and American Express), by company or personal check, or by wire transfers (an extra fee of
US $50 applies to wire transfers). Checks or money orders must be in US dollars, drawn on US banks, and

CANCELLATIONS AND SUBSTITUTIONS
Cancellations and substitutions are allowed. However, full registration fees will be
charged unless a cancellation notice is sent by email
(registration@csbcon.org) or fax (+1-650-941-2015)
on or before Monday, August 3, 2005. A $50 handling fee will be applied to all canceled registrations. Fees
cannot be refunded for registrations canceled after the deadline.
For substitutions send email with the name of the replacement to
registration@csbcon.org.
